Podem dir que entre el 20-30% de dones d&#8217;edat mitjana i entre el 30-50% de dones majors de 65 anys declaren tenir incontin\u00e8ncia urin\u00e0ria.<!--more--><\/p>\n<p>La <strong>incontin\u00e8ncia urin\u00e0ria<\/strong> \u00e9s la p\u00e8rdua involunt\u00e0ria d&#8217;orina que condiciona un problema social i higi\u00e8nic, afecta m\u00e9s a la dona que a l&#8217;home i la probabilitat de patir-la augmenta amb l&#8217;edat, encara que no \u00e9s una conseq\u00fc\u00e8ncia exclusiva o natural de l&#8217;edat.<\/p>\n<p>Podem classificar la <strong>incontin\u00e8ncia urin\u00e0ria<\/strong> en:<\/p>\n<ul>\n<li><strong>Incontin\u00e8ncia Urin\u00e0ria d&#8217;Esfor\u00e7<\/strong> (IUE): si hi ha una p\u00e8rdua involunt\u00e0ria d&#8217;orina associada a un esfor\u00e7 f\u00edsic que provoca un augment de la pressi\u00f3 intraabdominal, com en tossir, en esternudar, en aixecar pes o en c\u00f3rrer.<\/li>\n<li><strong>Incontin\u00e8ncia Urin\u00e0ria d&#8217;Urg\u00e8ncia<\/strong> (IUU): si hi ha una abrupta i sobtada urg\u00e8ncia d&#8217;orinar que no pot posposar-se, per tant hi ha incapacitat per contenir l&#8217;orina el temps suficient per anar al bany.<\/li>\n<li><strong>Incontin\u00e8ncia Urin\u00e0ria Mixta<\/strong> (IUM): si hi ha una p\u00e8rdua involunt\u00e0ria d&#8217;orina associada tant a la urg\u00e8ncia com a l&#8217;esfor\u00e7.<\/li>\n<\/ul>\n<p>Hi ha altres tipus d&#8217;<strong>incontin\u00e8ncia urin\u00e0ria<\/strong> per\u00f2 no s\u00f3n tan habituals.<\/p>\n<p><strong>Els m\u00fasculs del s\u00f2l p\u00e8lvic s\u00f3n importants perqu\u00e8 <em>s\u00f3n els encarregats de la contin\u00e8ncia urin\u00e0ria<\/em><\/strong>. Per tant, el tractament per a la incontin\u00e8ncia urin\u00e0ria hauria de basar-se a millorar la funci\u00f3 dels m\u00fasculs del s\u00f2l p\u00e8lvic.<\/p>\n<p>Entre les disfuncions m\u00e9s habituals del s\u00f2l p\u00e8lvic trobem la <strong>incontin\u00e8ncia urin\u00e0ria<\/strong>, la <em>incontin\u00e8ncia fecal<\/em>, els <em>prolapses genitals<\/em> (descens dels \u00f2rgans) i la <em>dispar\u00e8unia<\/em> (dolor durant les relacions sexuals).<\/p>\n<p>El tractament de fisioter\u00e0pia utilitza t\u00e8cniques destinades a enfortir la musculatura del s\u00f2l p\u00e8lvic com per exemple: <em>exercicis del s\u00f2l p\u00e8lvic<\/em>, <em>electroestimulaci\u00f3<\/em>, <em>biofeedback<\/em>, cons vaginals, <em><a href=\"http:\/\/www.nlm.nih.gov\/medlineplus\/spanish\/ency\/patientinstructions\/000141.htm\" target=\"_blank\">exercicis de Kegel<\/a><\/em>, <em><a href=\"https:\/\/www.activasalut.com\/ca\/9-beneficis-per-realitzar-exercicis-hipropressius\/\" target=\"_blank\">exercicis hipopressius<\/a><\/em>, <em>ter\u00e0pia manual<\/em>, etc. Aquestes t\u00e8cniques i exercicis produeixen una disminuci\u00f3 dels episodis d&#8217;incontin\u00e8ncia, un augment de la for\u00e7a, de la resist\u00e8ncia i de la coordinaci\u00f3 de la musculatura del s\u00f2l p\u00e8lvic i una millora de la qualitat de vida de les pacients.<\/p>\n<p>\u00c9s important la supervisi\u00f3 i la revisi\u00f3 setmanal de les pacients amb <strong>incontin\u00e8ncia urin\u00e0ria<\/strong>, ja que la restauraci\u00f3 del to i de la funci\u00f3 muscular succeeix entre 20-60 dies despr\u00e9s de comen\u00e7ar el tractament. Es recomana entrenament dels m\u00fasculs del s\u00f2l p\u00e8lvic per almenys 3 mesos.<\/p>\n<p>La rehabilitaci\u00f3 del s\u00f2l p\u00e8lvic ha de ser el primer tractament d&#8217;elecci\u00f3 per a la incontin\u00e8ncia urin\u00e0ria, abans de passar per una intervenci\u00f3 quir\u00fargica, ja que despr\u00e9s del tractament de fisioter\u00e0pia per a la<strong> incontin\u00e8ncia urin\u00e0ria<\/strong> un 81-84% de dones milloren i estan satisfetes amb el tractament.<\/p>\n<p>El tractament quir\u00fargic per a la <strong>incontin\u00e8ncia urin\u00e0ria<\/strong> solament hauria de considerar-se quan el tractament de fisioter\u00e0pia fracassa o falla, o si el pacient decideix no dur-ho a terme.<\/p>\n<p>Normalment les dones amb<strong> incontin\u00e8ncia urin\u00e0ria<\/strong> triguen uns 3 anys a buscar atenci\u00f3 m\u00e8dica i fisioter\u00e0pia des de l&#8217;inici del problema. No obstant aix\u00f2, el llarg per\u00edode de temps entre l&#8217;inici de la p\u00e8rdua urin\u00e0ria i la recerca d&#8217;ajuda no solament comporta un empitjorament de la p\u00e8rdua en si sin\u00f3 tamb\u00e9 de la qualitat de vida de les pacients.<\/p>\n<p><strong>Hem de ser conscients que la incontin\u00e8ncia urin\u00e0ria \u00e9s tractable i no considerar que \u00e9s un resultat natural del proc\u00e9s d&#8217;envelliment<\/strong>.<\/p>\n<p>Laura Baixarias<\/p>\n<p>Fisioterapeuta<\/p>\n","protected":false},"excerpt":{"rendered":"<p>En l&#8217;actualitat hi ha un alt percentatge de dones que pateixen incontin\u00e8ncia urin\u00e0ria.
